This policy explains how information submitted through this website is used. It applies to the website only — clinical records for patients of the practice are maintained separately in a secure electronic health record system.

Information You Submit Through This Website

The appointment inquiry form on the contact page asks for your name, email, phone, preferred contact method, whether you are a new or existing patient, and an optional non-clinical note. This information is used for one purpose: so Suzanne can personally review your inquiry and respond to you. It is not sold, shared, or used for marketing.

The form intentionally does not collect medical details. Date of birth, reason for seeking care, medication history, insurance details, and other clinical intake information are collected later through the practice's secure patient portal — not through this website. Form submissions are transmitted over an encrypted (HTTPS) connection and delivered to the practice through the website's form service.

This Form Is Not for Emergencies

The website and its forms are not monitored continuously. If you're experiencing a mental health emergency, call 988 or 911 now.

Patient Records Are Kept Separately

This website does not store your medical records. Once you become a patient, your clinical information is maintained in the practice's electronic health record and secure patient portal (TherapyNotes). Patients receive the practice's Notice of Privacy Practices, which governs how protected health information is used and disclosed.

Website Analytics

This site uses Google Analytics to understand how visitors use the website (for example, which pages are visited and for how long). Analytics data is aggregated usage information — it is not linked to your appointment request or any clinical record. You can limit analytics cookies through your browser settings or Google's opt-out tools.

Third-Party Links

Pages on this site link to outside resources (for example, the patient portal, government health resources, and directory profiles). Those sites have their own privacy practices, which this policy does not cover.
